Help Improve Hospital Discharge Summaries

Are you a care home manager or do you work in a non-nursing residential care home in England?

Do you read discharge summaries as part of your role?

We need your help to make hospital discharge summaries easier to understand and use. These summaries are important—they tell care homes what support and medicine a resident needs after leaving the hospital.

There are two ways you might get involved. We’re looking for:

  1. Care home managers to help us recruit residents for our steering committee
  2. Adult social care professionals who want to join the committee to share their ideas.

Your insights will shape how hospitals communicate with care homes. We will support you throughout the experience with flexible ways to be involved.
